I want to run a web proxy on my server, doesn't need to be fancy just direct my browser to a page where i input another url. is there a software package that can do this?

Yup, though phproxy is discontinued and no longer maintained. You could try "Glype" instead which is actively developed and have better support for myspace, facebook etc (if you use such stuff)

If phproxy fill your needs there's not really any need to change though.. ;)

I see that you are using phpproxy. Recommended you download glype script which they support both youtube and myspace.
