I personally use http://totalchoicehosting.com and have for a number of years. Another good recommendation is http://jaguarpc.com which a friend of mine uses.

Both provide you with PHP and MySQL and both have very good uptimes and tech support.

If you want very cheap, lots of features but occasional downtime try www.dreamhost.com - you can find coupons to get the first year free.

If you want the best, try pair.com (they actually host parts of www.php.net) but are quite expensive.
